Without crystal confirmation, the 2015 criteria score clinical features (pattern of joint involvement including the first metatarsophalangeal joint, inability to bear touch, redness, time course of episodes, tophi), serum urate, synovial fluid results, and imaging evidence of urate deposition (ultrasound double-contour sign or dual-energy CT) or gout-related joint damage.
